District 205 Sports Physical Night

Any students (GHS, Lombard, Churchill, Costa) interested in participating in sports next year, please don’t miss out on the annual GHS Booster Sports Physical Night.

There is a $10 fee due at the time of the exam.

Parents and students much complete and sign the Health History information on the physical form before the physical night. (No exceptions!)

Physical forms are available at the GHS Front Office as well as at the Athletic Office.

Please enter through the front doors.  Last physical given at 7:15pm.

(This exam does not meet state requirements for incoming 6th Graders and Freshmen)
